You can register at reception. Collect the registration forms from reception.
We ask all new patients to please bring two forms of ID for each adult family member; one of which to be a picture and one with your up to date address. We also request that you provide us with your NHS number. However, we are still able to register you if you do not have access to these forms of identification or proof of address, as per national guidance. If this is the case, or you need assistance or support in completing these forms, then please discuss with a member of the reception team. We may also be able to signpost you to our Social Prescriber (who supports patients with their well-being more generally).
If you are a carer or you have a carer please tell us when you register so we can note it on our records and provide information re Carers Leeds for additional support. This does not apply to those who are simply looking after children within the family. Please ask the receptionist for further advice if you are unsure.
We offer voluntary HIV Hepatitis B&C screening for newly registering adult patients.
If you would like to be screened for these conditions, please book an appointment with one of the nursing team for a blood test.
We register patients from Leeds 9, 14 and 15

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
