GP Learning Disability Register leaflet - NHS England has published a leaflet about the GP Learning Disability Register. Parent Carer Leaflet
The leaflet helps people think about whether their child or someone they care for, could be
considered to have a learning disability. If so, they should be on their GP practice’s Learning
Disability Register, to help them get the help they need.
This information can be used by staff in GP practices, health professionals, schools and
colleges and charities, as well as parents and carers of children who have a learning
disability.

Safe Surgeries
Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address, you can still register. Immigration status or nationality don’t matter - reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved.

Pharmacy blood pressure check finder tool
A new service finder on NHS.uk allows healthcare professionals and patients to search for and find a pharmacy that offers free blood pressure checks. The tool explains who is eligible to go to a pharmacy to get their blood pressure checked and can be searched using a postcode, with the nearest pharmacy listed first. Blood pressure conditions pages on NHS.UK have also been updated to signpost to the blood pressure pharmacy finder.
All pharmacy contractors offering the service should ensure their pharmacy profile on profile manager is up to date and indicates that they provide the service.

Care Navigation
What is Care Navigation ? When you feel ill and you ring your GP practice, you want to see someone as soon as possible to help make you feel better.
Sometimes, the GP (doctor) might not be the best person to see. It could be the GP practice pharmacist, the physiotherapist, a mental health worker or a practice nurse. In some cases, your GP practice might not be the best place at all and it could be a community pharmacy or optician that is more appropriate.
For this reason, our receptionists (also know as care navigators) have been trained to help make sure you see the right person depending on your medical problem. They may ask you some very brief questions about your symptoms but will not try to diagnose you or offer medical advice.
You can help by providing some information so they can offer you an appointment as quickly as possible with the right person in your GP practice. This may be with your GP or another member of the practice team.

Patient Participation Group
A Patient Participation Group (PPG) is a group of volunteer patients, carers and GP practice staff who meet to discuss practice issues and patient experience to improve the service.
If you would like to:-
- give patients and practice staff the opportunity to meet and discuss topics of mutual interest
- provide a means for patients to become more involved and make suggestions about the healthcare services they receive
- explore issues from patient complaints and patient surveys, contribute to action plans and help monitor improvements
- contribute feedback to the practice on the National Patient Survey / Friends & Family test to propose developments or change
- support health awareness and patient education

Online Access to Patient Record
Important information about online access to patient records:
The NHS wants to give people better ways to see their personal health information online. We know that people want to be able to access their health records. It can help you see test results faster. It also lets you read and review notes from your appointments in your own time.
From 1 November 2022, some patients over the age of 16 may be able to see new information from your doctor (GP) on your online account, though the NHS app, website or another secure record access app. This information may include notes from your appointment as well as test results and letters.
You may not be able to view this information at times: this will depend on your practice policies and security systems. In addition, before giving access to records, your doctor may want to talk to you first, for example, about test results or to make sure that having access is of benefit to you, as there may be sensitive information on your record. They may also want to talk to patients who might benefit from support to make a decision about access to their records. If you have any concerns or if you need support or more information, please talk to practice staff or your doctor.
These changes only apply to people with online accounts. If you do not want an online account, you can still access your health records by requesting this information through reception. The changes also only apply to personal information about you. If you are a carer and would like to see information about someone you care for, speak to reception staff.
It is very important that you protect your login details to the NHS App, website or other online services. Don't share your password with anyone as they will then have access to your personal information. You should also make sure you log off as soon as you have finished viewing your record, as this may not happen automatically for some apps.
If you do not want to see your health record, or if you would like more information about these changes, please speak to your doctor or your practice staff.

Our dedicated team are here to treat those minor ailments that occur as well as providing specialist management of long-term conditions and clinics covering a wide range of healthcare issues. The technology also means you can now do a lot of things from the comfort of your home such as order a repeat prescription or cancel an appointment.

Once registered, patients and healthcare professionals work together to ensure the most appropriate care is provided. This partnership philosophy extends even further and our active patient group exists to make sure that patient needs and the practice offering are always heading in the same direction.
Access
All consultation rooms are on the ground floor with access for wheelchairs, additionally there are disabled toilets facilities within the surgery.
Access Information Standards
The accessible information standard aims to make sure that disabled people have access to information in a way they can understand and any communication support needs they may have. This includes information in different formats, such as large print, easy read, via email or letter. In addition, we can support people's communication needs by offering support from British Sign Language (BSL) interpreters, deaf/blind manual interpreters or an advocate. Please let us know if you have any special communication needs so we can make sure we can meet your needs.
