POLITE NOTICE TO ALL PATIENTS

A Zero Tolerance Policy towards violent, threatening and abusive behaviour is now in place throughout the NHS.  At no time will any such behaviour be tolerated within the practice.

Please be respectful to all staff.

Requests for Certificate of Absence from the workplace relating to Covid-19 may fall into several categories as per attached:-

Coronavirus had led to a number of changes for your practice but we're very much open and consulting with patients at Foundry Lane Surgery.  To help keep everyone safe and to minimise the risk of infection, we're currently doing most consultations by phone or video; however, if we need to see you, then we will ask you to come to the surgery or arrange a home visit.

We're also carrying out routine and preventative care such as cervical screening tests, children's immunisations and reviews for people with existing health conditions.  If you receive a reminder about any of these, please make an appointment - you'll be seen in a safe environment.  When you book an appointment, we will ask you a set of questions to make sure you don't have symptoms associated with coronavirus:

- a high temperature

- a new continuous cough

- loss or change of sense of smell or taste

If you do have any of the above symptoms, the government requirement is that you MUST self-isolate for 10 days.  This means staying at home and only leaving your home to get tested.

The government requirement is also that ANYONE living with you will need to stay at home for 14days.

Please follow these steps to keep everyone safe.

If your symptoms are serious, or get worse, contact NHS 111 and they will advise you what to do. 

Ordering you medication

If you need to order medication from the surgery, order it online (speak to Reception if you do not have on-line access yet) or emails us: foundry.lane@nhs.net or send in your request by post.  Your chemist may also be able to order it for you although this is becoming less routine and they may ask you to order it yourself.  To keep people safe we are trying to minimize footfall in the surgery and limit this to when it is absolutely necessary, so please try and use the options as above when ordering medication rather than coming down to the surgery.  Thank you.

We are dealing with all other requests as fast as we can, but please bear with us if some of these take a little longer than usual.

Your NHS is here for you in an emergency.  Please remember that if you, or someone living with you, is experiencing a medical emergency such as a heart attack, stroke or severe bleeding you must ring 999 or go to your nearest accident and emergency department.

Flu Information

Please Note:-

Patients in the 50-64 year old age group will not be vaccinated until November and December, providing there is sufficient vaccine, and no appointments will be offered for this age group until then.  This is to ensure that those who are most at risk are vaccinated first.  If you are 50-64 and you are in one of the other groups which is eligible for the flu vaccination, for example you have a health condition which puts you at risk from the flu, you will be invited earlier.

Face Covering exemptions - The Practice does not issue exemption letters for the use of face masks / coverings. 

Please see information set out by the Government:- An exemption card had been developed for people who are unable to wear face coverings in shops and other indoor public spaces.  West Yorkshire Metro have also developed one for public transport.
